Your new role:

As a Senior Social Worker within the Dementia Outreach Service (DEMOS), you will provide expert clinical Social Work services to people living with dementia and their families and carers across the community. You will use your clinical expertise to manage complex psychosocial needs, provide advice to colleagues, support less experienced clinicians and contribute to improving dementia care across the service.

  • Provide senior, evidence-based Social Work assessment, intervention and care planning for people with complex dementia-related needs.
  • Provide clinical advice, supervision and education to less experienced clinicians, students and support staff.
  • Work collaboratively with an interprofessional team, consumers, carers and community organisations to support person-centred outcomes and effective care transitions.
  • Contribute to quality improvement, research, service development and clinical practice initiatives that enhance Social Work and dementia services.

About you:

We are looking for an experienced Social Worker who brings strong clinical capability, sound professional judgement and a passion for supporting people living with dementia in community settings.

  • Demonstrated senior-level expertise in Social Work clinical practice, particularly within community health.
  • Experience supporting people experiencing cognitive impairment, dementia and responsive behaviours, including in home and residential care settings.
  • Strong communication, collaboration and conflict resolution skills, with the ability to provide clinical advice to multidisciplinary colleagues and stakeholders.
  • Demonstrated ability to provide clinical supervision and education and contribute to service improvement initiatives.
