We're excited to welcome an Aboriginal and Torres Strait Islander Nurse Navigator (identified) to Extended Care Services.

The Nurse Navigation Service is a state-wide model of care designed to provide holistic support to patients with complex multiple health conditions e.g., chronic disease, mental health, frail and aged who require an expert level of healthcare coordination. Nurse Navigators work across system boundaries to actively improve health literacy in close partnership with patients and families and other health and allied health services to healthcare experience and quality of life. A proactive and integrated approach is taken to ensure patients receive the most appropriate services where and when they need them.

Whilst this position will work closely with Aboriginal and Torres Strait Islander consumers as a priority, all Nurse Navigators are expected to work across the Nurse Navigator program.

Key Responsibilities include:

• Lead the achievement of positive patient outcomes by:
  • Provide a central point of communication and engagement to ensure optimal care and coordination of services along a patient's entire healthcare journey are being met – this includes culturally appropriate holistic patient and family centered care. Educate and improve health literacy, to enable patients to self-manage, participate in decisions about their health care, and improve their own health outcomes.
  • Adhere to relevant clinical guidelines relating to Aboriginal and Torres Strait Islander consumers (of various ages) and follow ‘close the gap' guidelines on a local and national level to improve systems to enhance overall health and wellbeing.
  • Ensure that the Nurse Navigator Program's key performance indicators (KPIs) are met.
  • Applying expert clinical nursing knowledge and skills and coordinating clinical practice across the healthcare continuum including facilitating pathways and access for patients to the most appropriate services in a timely manner.

Remuneration value up to $174 208 p.a, comprising salary rates $70.92 - $77.00 p.h, employer contribution to superannuation (12.75%) and annual leave loading (17.5%) (NG7) (part time position available, 45.6 hrs p.f.)
