About the role
As a Nurse Practitioner within the Older Person Emergency Network (OPEN), you will provide advanced nursing care to frail older people, geriatric patients and individuals requiring palliative care across Residential Aged Care Facilities and community-based settings. Working at an advanced scope of clinical practice, you will utilise specialist expertise in assessment, diagnosis, care planning, treatment and evaluation to deliver high-quality, patient-centred care and improve health outcomes for older people.
You will work collaboratively with multidisciplinary healthcare teams to lead and support innovative nurse-led models of care, including emergency substitution services, community outreach and aged care initiatives. The role provides opportunities to influence service development, mentor clinicians, contribute to research and quality improvement activities, and support the delivery of responsive, evidence-based care that reduces avoidable hospitalisations and enhances patient experiences.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Provide advanced clinical assessment, diagnosis, care planning and management for older persons, geriatric patients and individuals requiring palliative care across community and residential aged care settings.
- Deliver specialist nursing interventions and clinical leadership in emergency care, geriatric care and palliative care using evidence-based practice and advanced clinical decision-making.
- Work collaboratively with multidisciplinary teams, community providers and healthcare stakeholders to coordinate care and optimise patient outcomes.
- Lead quality improvement, service evaluation and research activities that support best practice, innovation and enhanced models of care for older people.
- Provide mentorship, education and support to Nurse Practitioner candidates and other clinicians while contributing to professional development and workforce capability.
