About us:

The Specialist Outpatients Department (SOPD) is a large, diverse outpatient service spanning three levels of Rockhampton Hospital and providing specialist care across a broad range of clinical areas, including Cardiology, Dermatology, Ear, Nose and Throat, General Surgery, General Medicine, Gynaecology, Ophthalmology, Orthopaedics, Paediatrics and Urology.

The Project:

The Introduction of Criteria for Transfer of Care (TICTOC) Cardiac Program is a statewide quality improvement initiative supporting specialist outpatient services to improve how cardiac patients transition from specialist care to primary care.

The program introduces evidence-informed discharge criteria and structured clinical handover processes, enabling patients who no longer require specialist review to be safely transferred back to their GP. This helps improve access to specialist cardiology services, increase outpatient capacity, support care closer to home and enhance the patient experience.

Delivered in partnership with clinicians, nurses, consumers, GPs and other key stakeholders, the program aims to create a sustainable model that can be adopted across Hospital and Health Services and potentially expanded to other specialties.

About the role:

• Lead the TICTOC Cardiac Program at Rockhampton Hospital, driving implementation of the framework and supporting its adoption across participating sites.
• Provide advanced clinical and project leadership, using clinical expertise to guide outpatient discharge, transfer of care, service redesign and change management.
• Drive project delivery and evaluation, including project planning, governance, risk management, data analysis, reporting and monitoring outcomes against agreed objectives.
• Embed sustainable change, developing education, resources and implementation strategies that support evidence-based practice, improve patient flow and access to specialist care, and enable the TICTOC model to be scaled across services.

About you:

• Current Registered Nurse registration with AHPRA, with demonstrated experience and advanced knowledge in cardiac nursing practice.
• Expertise in evidence-based practice and quality improvement, with a commitment to evaluating outcomes, identifying opportunities for improvement and supporting best practice.
• Exceptional collaboration and communication skills, with the ability to work effectively with patients, carers, multidisciplinary teams and key stakeholders while demonstrating integrity, empathy and respect.
• Commitment to continuous improvement and professional development, including strengthening cardiac clinical expertise, supporting effective teams and contributing to quality frameworks that improve patient outcomes and the delivery of care.

What we offer:

Total monthly remuneration value of up to $14,517 p.m. is comprised of:

• An hourly rate between $70.92 - $77.00 p.h.
• 12.75% employer superannuation contribution
