Your new role:

Join the Rehab@Home team at Metro South Health and make a meaningful difference by supporting clients to achieve their best possible health outcomes in their own homes and community. Working as part of a multidisciplinary team, you will provide safe, compassionate and person-centred nursing care, with a focus on rehabilitation, restorative care and supporting independence.

In this role, you will:

• Deliver comprehensive nursing care including assessment, planning, implementation and evaluation.
• Provide community-based care such as wound management, catheter care and medication administration.
• Work collaboratively with nursing, allied health, clients and families to achieve person-centred outcomes.
• Educate clients and carers to build confidence in self-care and ongoing health management.
• Apply critical thinking and clinical judgement to prioritise care and respond to changing needs.
• Contribute to quality improvement, evidence-based practice and positive patient outcomes.
• Work with increasing autonomy while being supported by experienced senior clinicians.

About you:

You are a compassionate and motivated Registered Nurse who enjoys working collaboratively and independently, and who is passionate about helping people achieve greater independence in their own community. You will bring:

• Current registration with the Nursing and Midwifery Board of Australia (NMBA) and AHPRA.
• A tertiary nursing qualification such as a Bachelor of Nursing or equivalent.
• Clinical knowledge and skills relevant to, or transferable to, community nursing.
• Strong assessment, care planning, clinical decision-making and communication skills.
• A commitment to person-centred care, evidence-based practice and continuous learning.
• The ability to work effectively within a multidisciplinary team.
• A current appropriate licence endorsement to operate a Class C motor vehicle.
• A willingness to participate in an on-call roster.
