About the work unit / facility

The Adult Acute Mental Health Inpatient Unit (AAMHIU) is a 38-bed acute inpatient service located at Townsville University Hospital. The unit provides compassionate, short-term intensive treatment and support for individuals experiencing acute episodes of mental illness, offering a supportive environment focused on recovery, stabilisation and wellbeing when intensive inpatient care is required.

AAMHIU delivers care across two clinical areas:

  • Low Dependency Unit (LDU) – a 30-bed unit for consumers requiring acute inpatient care within a less restrictive environment.
  • High Dependency Unit (HDU) – an 8-bed unit designed for consumers requiring higher levels of observation, support and therapeutic intervention.

Competitive salary and benefits:

We offer attractive remuneration packages and generous leave entitlements.

  • Base salary range: $4390.00 to $4700.90 p.f. (Nurse Grade 6, Band 1)
  • 4 to 6 weeks annual leave with 14%-17.5% loading

About your opportunity

As the Discharge Coordinator, you will work collaboratively with the Clinical Nurse Consultant (CNC) and Shift Coordinator to coordinate and facilitate discharges from the AAMHIU. This includes supporting the CNC and Shift Coordinator to address identified barriers to timely discharge, ensuring the best possible outcomes for consumers and the optimal use of organisational capacity and resources.

As a valued member of the multidisciplinary team, you will work collaboratively with consumers, families and carers. You will support and participate in reviews of consumers who are outlied to other areas of THHS, as well as consumers on overnight leave, to ensure safe care and timely discharge from the AAMHIU. You will also ensure warm handovers are completed and all relevant documentation is maintained to support continuity of care.
