About the role

As a Senior Social Worker, you will play a vital role in delivering advanced, person-centred social work services that support consumers, families and carers across a diverse range of healthcare settings. Working within multidisciplinary teams, you will provide high-level psychosocial assessment, intervention, care coordination and discharge planning to individuals experiencing complex health, disability, ageing, rehabilitation, palliative care and chronic disease management needs. You will exercise a high degree of professional autonomy, applying expert clinical judgement to support positive patient outcomes while navigating complex social and healthcare challenges.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Exercise independent professional clinical judgement in providing Social Work clinical services of a complex nature where principles, procedures, techniques, or methods require expansion, adaptation or modification, without the need for direct regular clinical practice supervision and with infrequent practice supervision using valid and reliable evidence.
  • Apply clinical evidence to practice supporting continuous improvement of quality service delivery and participate in clinical research activities within the Social Work and multi-disciplinary team, as required.
  • Utilise high level communication and interpersonal skills to achieve optimal patient and service delivery outcomes.
  • Provide high level clinical advice to diverse stakeholders including professional and operational supervisors, relevant service managers, medical and allied health regarding service delivery, equipment, technology and the prioritisation and development of social work clinical services.
  • Provide clinical leadership managing complexity related to psychosocial care needs of consumers and their families/carers, and advocate to ensure that legislative, ethical and professional standards are maintained.
