Your new role:

Join the Pathways Team to support complex long-stay patients across Metro South Health. Provide social work expertise, support discharge planning and improve patient outcomes.

In this role, you will:

  • Deliver evidence-based social work services and complex case management to long-stay inpatients across acute and subacute settings.
  • Work collaboratively with multidisciplinary teams to identify and address barriers to timely discharge, including NDIS, aged care and legal matters.
  • Provide clinical advice and support complex decision-making using your expertise in disability, aged care, guardianship and psychosocial assessment.
  • Contribute to service development, quality improvement, clinical education and evidence-based practice.
  • Provide supervision, training and support to less experienced clinicians, students and clinical assistants.
  • Build effective relationships with patients, carers, health professionals and external agencies to achieve patient-centred outcomes.

About you:

Experienced social worker with strong clinical judgement, communication and collaboration skills, committed to delivering high-quality care and positive outcomes for complex patients.

To be successful in this role, you will bring:

  • A tertiary qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW).
  • Demonstrated high-level knowledge and experience in social work clinical services, complex case management and discharge planning.
  • Strong understanding of disability, aged care, guardianship, psychosocial assessment and complex system navigation.
  • Excellent communication, negotiation, conflict resolution and stakeholder engagement skills.
  • Demonstrated ability to prioritise and manage complex workloads across a busy clinical environment.
  • Experience providing supervision, training and support to less experienced practitioners and students.

The Queen Elizabeth II Jubilee Hospital (QEII) is a medium metropolitan adult facility providing a range of inpatient and outpatient services including a 24-hour Emergency Department. We provide a range of services, including surgery, cardiology, endoscopy, orthopaedics, gynaecology, urology, geriatric, respiratory and emergency medicine.

We're making our hospital bigger and better to make it easier for our growing community to get the health care they need. We'll have more beds, medical wards and expanded intensive care and emergency services planned to be opened by 2027.
