- Base salary range: $70.92 - $77.00 p.h.

About your opportunity
Join the Frailty Intervention Team (FIT) and help improve the healthcare journey of frail older people across emergency, inpatient and residential aged care settings.
As a Clinical Nurse Consultant, you will provide specialist gerontic assessment, clinical leadership and care coordination through three key service streams:
- Frailty Emergency Department Investigation (FEDI): Partner with the Emergency Department team to identify frailty early, assess clinical risks, establish goals of care, and support timely clinical decision-making, care planning and disposition.
- FIT Triage: Provide expert telephone triage, clinical advice and care coordination for Residential Aged Care Home (RACH) residents experiencing acute health concerns, supporting access to the most appropriate care pathway.
- FIT Inpatient Consultation: Monitor admitted RACH residents, promoting continuity of care, reinforcing goals of care, integrating geriatric principles into acute care and facilitating safe, timely discharge planning.
