Your opportunity:
- The Integrated Care Nurse Navigator (Quality and Innovation) is a clinical leader, accountable at an advanced practice level for the development, coordination and implementation of strategic organisational strategies and initiatives within the Integrated Care Services utilising quality, innovation and project management methodologies.
- The Integrated Care Nurse Navigator (Quality and Innovation) incumbent demonstrates sound knowledge of contemporary nursing practice and theory and provides clinical leadership, advice and advocacy to a multidisciplinary team and is responsible for the facilitation and provision of quality and cost effective nursing and/or midwifery services which are consistent with contemporary standards, guidelines, relevant legislation and best practice in the delivery of hospital avoidance strategies, chronic condition pathways and innovative hospital in the home initiatives which aim to improve the health and wellbeing of ambulatory care clients by delivering the right care in the right place at the right time.

Our offer:
You will receive a competitive remuneration package value up to $174,208 p.a., comprising salary between $140,631.69 - $152,684.94 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 7) (Permanent Full Time, 76 hours p.f.)
