Your new role:
As a Dietitian within the Rehab@Home service, you will provide high-quality, evidence-based dietetic care to clients across community and residential settings, working as part of an interprofessional team to support rehabilitation, restorative care and patient-centred outcomes.
- Deliver dietetic assessment, intervention and evaluation for an allocated caseload, applying clinical judgement and evidence-based practice within established protocols and under the guidance of senior clinicians.
- Work collaboratively with clients, families and carers, and a range of health professionals to develop and achieve person-centred goals, including undertaking care coordination responsibilities as required.
- Contribute to clinical education and supervision of students, allied health assistants and less experienced staff, with support from senior clinical staff.
- Participate in quality improvement, service development and research activities that enhance dietetic practice and promote safe, effective and patient-centred care.
About you:
We are looking for a compassionate and collaborative Dietitian who is committed to delivering high-quality care and can thrive in a dynamic, community-based rehabilitation environment.
- Demonstrated ability to provide high-quality, efficient, patient-centred and evidence-based dietetic assessment and management within a community-based restorative and rehabilitative model of care.
- Strong verbal and written communication skills, with the ability to work effectively within an interprofessional team and build positive relationships with clients, carers, families and other stakeholders.
- A commitment to ongoing learning and professional development, with the ability to develop clinical knowledge and expertise through active learning and engagement with the team.
- Demonstrated ability, or the capacity to rapidly develop the skills, to contribute to clinical education, student training and local service development and improvement initiatives.
