About the role
The Senior Pharmacist (Including Virtual Ward) delivers high-level clinical pharmacy services across Community and Oral Health facilities, ensuring safe, effective and patient-centred medication management across the continuum of care. Working within multidisciplinary teams, the role supports a diverse range of services including inpatient rehabilitation, Hospital in the Home, Virtual Ward, community-based care, transitional care programs and residential care settings.
The successful candidate will manage complex clinical caseloads, provide expert medication reviews and pharmaceutical care plans, participate in care planning and discharge processes, and contribute to medication safety initiatives. The role also supports service improvement, education, clinical governance activities and research while helping drive innovative pharmacy practices in community and virtual healthcare environments.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Provide advanced clinical pharmacy services, including medication reviews, pharmaceutical care planning and medication safety interventions across community and virtual healthcare settings.
- Deliver expert clinical advice and medicines information to patients, carers and multidisciplinary healthcare teams.
- Participate in care planning, case conferences, discharge counselling and continuity of care activities to support seamless transitions between hospital and community care.
- Contribute to clinical governance, quality improvement initiatives, medication usage evaluations and service development activities.
- Provide education, supervision and professional support to pharmacy students, interns and pharmacists while promoting evidence-based practice.
