About us:

Helping patients move through hospital care with the right support, at the right time. The Medical Care Coordination service brings together General Medical teams, hospital departments and community providers to create a more connected patient journey across Rockhampton Hospital.

By coordinating care, anticipating barriers and linking patients with the services they need beyond hospital, the service supports safe, timely and well-planned discharges, helping patients transition from hospital back into the community with confidence. Working across teams including medical, nursing, imaging, ambulance, primary and community health services, the service is all about connecting the dots around the patient and making their care journey as streamlined as possible.

About the role:

  • Lead advanced, person-centred nursing care for patients with acute and complex medical conditions, applying critical thinking and evidence-based practice.
  • Coordinate patient care and discharge planning for General Medical patients, supporting safe, timely and effective transitions from hospital to home or community care.
  • Provide clinical leadership across practice, education and research, promoting high-quality care, professional development and positive patient outcomes.
  • Champion quality improvement and innovation, working with the NUM to implement policy, practice changes and clinical innovations while supporting strong clinical governance and safety.

About you:

  • Registered Nurse with current NMBA/AHPRA registration; advanced clinical knowledge and experience in acute medical nursing is highly desirable.
  • Bring advanced clinical judgement and problem-solving skills, with the ability to assess, plan, coordinate and evaluate comprehensive, person-centred care.
  • Champion education, quality and innovation, contributing to audits, quality initiatives, policy and practice changes, while supporting the learning and development of others.
  • Demonstrate strong professional leadership and integrity, to improve patient outcomes and maintain high standards of safe, quality care.

What we offer:

Total monthly remuneration value of up to $7,346 p.m. is comprised of:

  • An hourly rate between $57.76 - $61.85 p.h.
  • 12.75% employer superannuation contribution
  • 17.5% leave loading
