About the role
The Advanced Carer Peer Worker works within the Mental Health Hospital in the Home (MH HiTH) multidisciplinary team at The Prince Charles Hospital, providing peer support, reassurance and practical assistance to consumers receiving care through the service. Drawing on lived experience as a carer of a loved one with mental ill-health, the role promotes hope, recovery and wellbeing while supporting consumers and their families throughout their recovery journey.
The successful candidate will use their lived experience to support recovery planning, advocate for consumers and carers, facilitate education and support activities, and collaborate with clinicians and community organisations to enhance recovery-oriented care. The role also contributes to service improvement, health promotion, consumer feedback initiatives and the ongoing development of lived experience practices within Metro North Mental Health.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Provide peer support and share lived experience to promote recovery, wellbeing and positive outcomes for consumers receiving Hospital in the Home services.
- Support consumers to identify strengths, develop recovery goals and participate in recovery and safety planning.
- Advocate for consumers, families and carers while providing practical assistance and support throughout the recovery journey.
- Collaborate with multidisciplinary teams and external organisations to promote recovery-oriented, person-centred care.
- Deliver and support lived experience education, health promotion activities and service improvement initiatives within MH HiTH.
