Charters Towers Hospital are seeking a Nurse Navigator to join the team within Complex and Chronic Care on a Permanent part-time basis 38 hours per fortnight and sits within Rural Service Group.

Eligible health workers who make the move (interstate or international) to work with us may receive up to $70,000 in incentive payments under the Queensland Health Workforce Attraction Incentive Scheme (conditions apply).

About your opportunity

The Nurse Navigator facilitates person-centred care, integrated care pathways through coordination of models of care and service access for patients with chronic conditions and/or complex healthcare needs. This facilitation is achieved by reducing fragmentation, mitigating barriers, educating, empowering, improving health literacy and supporting coordinated care through the creation of partnerships with care providers including local and primary care and non-government providers.

The Nurse Navigator will demonstrate clinical expertise, utilise case management principles and primary health care service alternatives, where clinically appropriate, with the aim of reducing avoidable hospital admissions and improving patient outcomes through early and proactive interventions.

Generous remuneration package value up to $82,348 p.a. comprising:

  • Salary range of $66.85 to $72.58 p.h. (Nurse Grade 7)
  • Annual leave with 17.5% loading
  • 12.75% employer contribution to superannuation.

Requirements

  • Bachelor of Nursing or equivalent certification relevant to the position is mandatory.
  • Proof of qualification, registration with the Australian Health Practitioner Regulation Agency (AHPRA) and possession of a current practising certificate.
  • Highly Desirable: Possession of a relevant postgraduate qualification in an appropriate field is highly desirable. Such experience may be discipline specific or have a service area focus.
  • Drivers Licence: This position requires the incumbent to operate a class C motor vehicle and an appropriate license endorsement to operate this vehicle is required. Proof of this endorsement must be provided prior to the commencement of duty.
  • This is a Vaccine Preventable Diseases (VPD) risk role: Hepatitis A/B, Measles, Mumps, Rubella, Varicella, and Pertussis.
  • Probation period may apply.
  • Pre-employment checks will be conducted.
