The Single Aged Care Assessment System is a multi-disciplinary team providing assessment services to older Queenslanders in the community. The assessment system aims to simplify and improve access to aged care services.

- Salary range: $2950.00 to $3280.70 p.f. (AO3)
- 4 to 6 weeks annual leave with 14%-17.5% loading
- 12.75% employer contribution to superannuation
- Plus Salary Sacrificing options may be available
About your opportunity
To provide non-clinical home-based face to face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool (IAT). Under the Single Aged Care Assessment System, Home Support Assessors will work with clients and their representative/s to establish goals specific to the client's needs and develops appropriate strategies and actions, including referrals to aged care and non-aged care services.
Requirements:
- Mandatory requirement of a Certificate III in Home Care or Aged Care or equivalent work experience.
- Highly desirable a Certificate IV in Health Service Assistance/Allied Health Assistance.
- Attend and complete all mandatory training as outlined in the National Training Strategy 2023
- Demonstrated experience in word processing, spreadsheet, database and email software, preferably Microsoft Office suite.
- This is a Vaccine Preventable Disease (VPD) risk role.
