What your day could look like
As a Coordinator Home Care, you'll play a vital role in supporting people to live safely, independently and with dignity in their own homes. Working closely with care workers, clinicians and the wider community, you'll turn coordinated planning into real, everyday impact for clients and families.
Every day, you'll contribute by:
- Coordinating personalised home care services that respond to client needs and goals
- Leading and supporting a large care workforce to deliver consistent, high‑quality care
- Building strong partnerships with clients, carers and health professionals
- Driving service improvements that strengthen safety, quality and client experience

You're a calm, capable leader who balances empathy with sound judgement. You take pride in seeing people supported well, teams working confidently, and services running smoothly. Guided by compassion, integrity and courage, you know how to bring people together and make thoughtful decisions that improve lives.
Here's what will help you thrive in this role:
- Experience coordinating community or aged care services, including care planning
- Confidence supervising and supporting staff in a service delivery environment
- Strong digital skills for managing records, rosters and service data
- A current driver's licence and willingness to travel as needed
You'll stand out from the crowd if you bring experience in rural or regional services and/or a relevant qualification in aged or community care.
