Every day, you'll contribute by:

  • Coordinating personalised home care that responds to changing client needs and supports quality of life
  • Leading and supporting a team of home care workers to deliver safe, reliable and compassionate care
  • Partnering with health professionals, carers and community services to achieve the best outcomes
  • Driving continuous improvement in service quality, processes and team capability

Let's talk about you!

You're a calm, capable leader who brings heart and purpose to your work. You balance people and performance with ease, take pride in supporting both clients and teams, and enjoy seeing care delivered well. You're motivated by making a real difference and feel proud knowing your leadership helps people live well at home. Guided by compassion, integrity and courage, you lead with dignity and embrace new ways to strengthen care in a regional setting.

Here's what will help you thrive in this role:

  • Experience coordinating community‑based or aged care services, including care planning and assessment
  • Confidence leading, supervising and supporting a diverse workforce
  • Strong organisational, communication and digital skills to manage rosters, records and reporting
  • A current driver's licence and ability to work flexibly, including on‑call arrangements

You'll stand out from the crowd if you have experience working in rural or regional communities and/or a relevant qualification in aged or community care.

At its heart, this role is about leading with compassion and integrity to improve lives and strengthen community care.
