What your day could look like
As Coordinator Home Care, you'll play a vital role in supporting older people to remain safe, independent and connected in their own homes. You'll work closely with clients, families and a multidisciplinary team to ensure care is well coordinated, responsive and delivered with dignity across the community.
Every day, you'll contribute by:
- Coordinating personalised home care that responds to changing client needs and supports quality of life
- Leading and supporting a team of home care workers to deliver safe, reliable and compassionate care
- Partnering with health professionals, carers and community services to achieve the best outcomes
- Driving continuous improvement in service quality, processes and team capability

Here's what will help you thrive in this role:
- Experience coordinating community‑based or aged care services, including care planning and assessment
- Confidence leading, supervising and supporting a diverse workforce
- Strong organisational, communication and digital skills to manage rosters, records and reporting
- A current driver's licence and ability to work flexibly, including on‑call arrangements
You'll stand out from the crowd if you have experience working in rural or regional communities and/or a relevant qualification in aged or community care.
