What your day could look like
As Coordinator Home Care, you'll help older people live safely and independently at home by leading high-quality community care services. From coordinating tailored support plans to guiding a team of care professionals, you'll play an important role in delivering positive experiences and outcomes for clients across the region.
Every day, you'll contribute by:
- Coordinating personalised services, care plans and resources that support clients to achieve their goals and maintain independence.
- Building strong partnerships with clients, carers, health professionals and community providers to deliver seamless care.
- Leading and supporting a team of home care workers to provide safe, high-quality and responsive services.
- Driving service improvements, quality outcomes and compliance practices that strengthen client care and team performance.

You're a people-focused leader who combines compassion with practical problem-solving. You build trust quickly, communicate with confidence and know how to bring teams together around a shared purpose. The work you do helps older people continue living with dignity, choice and connection in their own homes. You're not just coordinating services; you're creating meaningful outcomes for individuals, families and the broader community.
Here's what will help you thrive in this role:
- Experience coordinating community, aged care or client-centred support services.
- Strong skills in care planning, stakeholder engagement and responding to changing client needs.
- Experience leading, mentoring or supervising staff in a service environment.
- Confidence using digital systems, maintaining accurate records and managing competing priorities.
You'll stand out from the crowd if you have a qualification in Individual Support (Ageing) or a related field and/or experience working within government-funded aged care programs.
