    The Transition and Post Care Support Team will work directly with young people aged 15-21 years who have complex and high risk behavioral, emotional, psychological, disability and/or mental health needs and who are most at risk of homelessness as they transition to adulthood. The team provides direct support to young people to build adaptive skill aimed to improve the success of maintaining tenancies and co-tenancies – reducing the risk of homelessness.

    Whild providing high quality supports and advice on a range of tasks associated with their complex and high risk support requirements, there will be a clear focus on positive outcomes for the young person in achieving their goals, interests and aspirations.
